This form allows the District of Columbia Retirement Board (DCRB) to release your retirement information, on file with DCRB, under the terms of the District of Columbia Police Officers, Firefighters, and Teachers Retirement Plans (the Plans).

Upon receipt of a completed form, DCRB will respond within ten (10) to twenty (20) business days, depending on the nature of your request. Some requests may take longer, and you may be asked to provide additional identification information for verification purposes.

Note: This form must be signed by the payee, or the payee’s authorized representative. The validity of the legal relationship of the payee’s authorized representative must be previously approved by DCRB. You can have the validity of your legal relationship reviewed by submitting either the DCRB Durable Power of Attorney form or the court ordered document from when the representative was legally authorized.
