Members can submit their materials to DCRB in a variety of ways:
- Submit online from the the DCRB website.

- Note: Please submit all supporting documents with your enrollment form if you are adding or removing a dependent from your healthcare plan. Examples would be:
- Divorce
- Marriage
- Adoption
- Birth
- Adult disabled child
