Contribution Balance Request Form


Complete and submit this form if you request your contribution balance from the District of Columbia Retirement Board (DCRB). After receipt of this request, DCRB will respond to your request within ten (10) business days.

You can send the completed form and any required attachments by fax to (202) 566 -5001 or by mailing your documents to:
