If you were hired on or after October 1, 1987, you are eligible to enroll in a District of Columbia health insurance plan.
Note: Please submit all supporting documents with your enrollment form if you are adding or removing a dependent from your healthcare plan. Examples would be:
- Divorce
- Marriage
- Adoption
- Birth
- Adult disabled child
