To apply for a deferred retirement annuity beginning at age 55 under D.C. Code § 5-717, complete and submit this form to the District of Columbia Retirement Board (DCRB) Member Services Center. Processing your request may take from thirty (30) to ninety (90) days to complete.

Please note: This form is valid only for survivors of Police Officers or Firefighters who are enrolled in the Plan.
