The purpose of the W-4P form, officially known as the “Withholding Certificate for Periodic Pension or Annuity Payments,” is to ensure that the correct amount of federal income tax is withheld from your pension payments. To complete the W-4P form, you’ll need to provide the following information:
- Personal Information: Your name, address, Social Security number, and filing status (single, married, etc.).
- Total Number of Allowances: The number of allowances you are claiming, which affects the amount of tax withheld.
- Additional Withholding: Any additional amount you want withheld from each payment.
- Sign and Date: Your signature and the date you completed the form.
