Periodic Certification of Full-Time School Attendance Form

Monday, September 21, 2020

Individuals between the ages of eighteen (18) and twenty-two (22) who receive a survivor annuity as a student-child must file periodic certification to indicate that they are pursuing a full-time course of resident study or training in a recognized educational institution. The District of Columbia Retirement Board (DCRB) must verify that the child survivor has resumed school attendance within five (5) months after the end of the past school year. DCRB also needs to update the information on the student's plans for school attendance after the current school year. If you receive a survivor annuity as a student-child, you should complete the form below for each semester/quarter of enrollment.

You can send the completed form and any required attachments by fax to (202) 566 -5001 or by mailing your documents to:

DC Retirement Board
Member Services Center
900 7th Street, NW, Second Floor
Washington, DC 20001

If you have further questions, please contact the DCRB Member Services Center by clicking on the Ask Member Services link, or by phone at (202) 343-3272 Monday – Friday from 8:30 am to 5:00 pm.
