Surviving Spouse Change of Status - Police and Firefighter

Please complete this form if you are a surviving spouse of a deceased police officer or firefighter member who wishes to make a change of status. Once you have completed the form, mail, email, or fax it to the DCRB Member Services Center. The original form must be mailed to DCRB.

Please note: In accordance with D.C. Code §5-716(e)(1), to remain eligible to receive a surviving spouse benefit, you cannot remarry prior to age 55. See the form for more details.

Surviving Spouse Under a Qualified Domestic Relations Order (QDRO): In accordance with the District of Columbia Spouse Equity Act of 1988, D.C. Code §§1-529.01 et seq., to remain eligible to receive a surviving spouse benefit as a former spouse under a QDRO, you cannot remarry prior to age 55.
