Name Change After Marriage

If you change your last name after marriage and would like to notify DCRB of this change, you must submit the Name Change Notification form along with a copy of the legal documents establishing the name change for processing to occur.

  • Copy of your certified marriage certificate
  • Other proof of your identity (passport or unexpired driver's license)

Name Change After Divorce

If you would like to reclaim your maiden name after divorce, a copy of the legal documents establishing the name change must accompany this form for processing to occur.

  • Copy of the divorce decree
  • Other proof of your identity (passport or unexpired driver's license)
