Please complete this form if you're requesting information for your contribution balance with the District of Columbia Retirement Board (DCRB). Upon receipt of this request, DCRB will respond to your request within ten (10) business days.
Important: This document is only to be used in response to a request from DCRB to complete the information. The intent of the form is to confirm that the annuitant is receiving a monthly annuity amount and requires that the form be signed in the presence of a Notary Public.
Please complete this form if you are a surviving spouse of a deceased police officer or firefighter member who wishes to make a change of status. Once you have completed the form, mail, email, or fax it to the DCRB Member Services Center.
Follow the link and choose the DC D-4 Employee Withholding Allowance Certificate from the list, which you will use to designate your withholding allowances if you live in the District of Columbia.
